As a patient of Dome Home Care, you have the following rights and responsibilities, consistent with Medicare's Durable Medical Equipment Supplier Standards.
Your Rights
You have the right to:
- Be treated with dignity, courtesy, and respect.
- Receive service without regard to race, color, national origin, age, disability, sex, religion, or source of payment.
- Confidentiality of all information about your care, equipment, and billing — no one outside your care team may access it without your written consent.
- Be fully informed, in advance, about the equipment and service to be provided, including any modifications to your plan of service.
- Receive your equipment and service in a timely manner.
- Choose your home medical equipment supplier and make informed decisions about your care.
- Receive information in a language or format you understand, including interpreter services where needed.
- Have the manufacturer's warranty honored on equipment you purchase.
- Receive a clear, itemized explanation of your bill and your financial responsibility before service begins.
- Receive at least 30 days' notice of any change in charges, where applicable.
- Voice a grievance or complaint without fear of reprisal, discrimination, or termination of service.
- Be free from mental, physical, sexual, and verbal abuse, neglect, and exploitation.
- Access and request amendment of your health information.
Your Responsibilities
To help us provide you the best possible care, please:
- Provide complete and accurate medical, insurance, and contact information.
- Promptly notify us of any changes to your address, phone number, insurance, or physician.
- Promptly notify us of any equipment failure, damage, or change in your condition or physician's orders.
- Notify us when equipment or services are no longer needed.
- Use equipment as instructed and ask questions if instructions are unclear.
- Meet your financial obligations promptly, including copays and any balance not covered by insurance.
- Participate actively in your care plan and treatment decisions.
