Effective date: January 1, 2020. This notice describes how Dome Home Care may use and disclose your protected health information (PHI), and your rights regarding that information. Please read it carefully.
We use and disclose your PHI to provide your prescribed equipment and supplies, coordinate with your physicians and care team, bill your insurance or Medicare, and conduct quality assessment and improvement activities. We may share information with contractors and business associates who help us deliver your equipment, process billing, or service your devices — all of whom are required to safeguard your information appropriately.
Marketing communications and any sale of your PHI require your written authorization. We will never sell your health information.
We are required by law to maintain the privacy of your PHI, provide you with this notice describing our legal duties and privacy practices, and follow the terms of the notice currently in effect. We will abide by any state law that provides greater privacy protection than HIPAA.
We reserve the right to change this notice. Any revised notice will apply to PHI we already have as well as PHI we receive in the future, and will be made available on this page and at our office.
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
